Distress Centre Halton – $5,000 (Larger Grant)
Distress Centre’s Mission is to support people in our community so they can cope better with crisis, loneliness and emotional stress. They do not define crisis. They do not judge. Anyone can call day or night. The Telecheck program will provide outbound calls to those with mental health issues to decrease the possibility of decompensation, bridge services of the Canadian Mental Health Associations’ Community Outreach Team when needed, and decrease reliance on costly downstream interventions.
Connect with Halton patients who have been recently discharged from a hospital or who presented at an Emergency Department for a mental health concern to ensure that they are functioning well following discharge, and have made necessary connections with both formal community resources and informal support networks.
Reach out to older adults who are experiencing social isolation and loneliness to ensure that they are supported and connected to help them age well.
Support older adults to maintain their independence and provide caregiver support to those caring for older adults.
